ATHLETICS
Thank you for your interest in Okemos Middle School Athletics
Each student-athlete must have an athletic physical on file from April 15th of the current calendar year in order to try out per MHSAA guidelines. If you do not have one, we would suggest going to Urgent Care or Redi-Care and asking for the student-athlete rate. If you have any questions regarding physicals, check with the coach of that particular sport or the athletic director. Attached you will find the updated form necessary to present to CMS athletic office.
REGISTERING PHYSICAL FORMS
Physicals need to be turned-in using this link: Big Teams Student Central. Both parent and student need to create an account. It is suggested that the student-athlete sets up their account first and invites the parent’s account afterward. If you need assistance setting up an account, more information about the site can be found here. Athletic Pre-Participation Forms Getting Started Guide (Parent & Student).
ONLINE PAYMENT
Our athletic fee per student-athlete is $130; there is a family cap of $370 (if a family has children playing sports and/or attending middle school and high school). We ask that all fees be applied once you know that your child has made the team. If you have any questions regarding fees, scholarships, or payments, please go to the Registration Fee Information or contact the athletic director...

MHSAA Mandatory Concussion Return to Play Form
New Post-Concussion Return-to-Play Requirement: We'll need the written clearance from an MD, DO, PA or NP, signature and we also require both students and their parents/guardians before the student can return to activity. It is also required the Return to Activity & Post Concussion Consent Form be emailed or faxed to the MHSAA and kept on file at the school for seven years following a student's high school graduation. The same form signed by the medical professional must then be signed by the student and parents before the student may return to activity. Students who participate without the required approval are ineligible athletes. The form is on the MHSAA website - Health and Safety - Return to Activity & Post-Concussion Consent Form. The reverse side of the form has information for parents on the MHSAA Concussion Care Gap Insurance that covers deductibles, copays, and students with no insurance.
